ASI-NE’s Community Integration program provides credentialed Community Integration Specialists (CIS) to assist adults living with mental illness to build on their abilities and live successfully in the community of their choice. These services are provided with flexibility and on an as-needed basis.
The CIS and consumer (or guardian) will work together to identify needs and set goals in the areas of:
- Living skills
- Mental health wellness
- Development of social skills
- Development of coping skills
- Personal financial management
- Obtaining and maintaining appropriate housing
- Achieving educational and vocational goals
- General health management
- Accessing local transportation and legal resources
- Crisis intervention and management
- Achieving educational and vocational goals
The CIS will coordinate referrals and advocate for access to resources as well as monitor service provision and regularly review progress made toward achieving goals; success in this program will require active participation by the consumer and/or guardian.
Community Integration Services are available to individuals residing the Kennebec, Oxford, Franklin, and Somerset counties. All participants must satisfy the eligibility requirements of Section 17.
