The Jay home is located within walking distance of downtown Livermore Falls, which has a grocery store, drug store, hair salon, library, several churches, a dental office, and medical facilities close by.  It is also within an hour’s drive to Farmington, Augusta and the Lewiston/Auburn area.

The home is a Level I licensed facility that meets all Home and Community-Based Standards. designed for individuals with a primary diagnosis of an intellectual disability, who may also have co-occurring symptoms of mental illness which would require a mild to moderate level of support. Individuals have a lot of potential for learning independent apartment living. It has 24-hour staffing present in the building.

The building has four apartments, one apartment serves as staff office and the other three apartments are for individuals seeking Section 21 support services. They may decorate the space according to the occupants’ own tastes. Each apartment has its own kitchen, living room and bedroom that the person living there is responsible for with staff support. There are laundry facilities in the basement. The individuals in this program need to be able to function without constant staff presence.

The direct care staff in this program assists everyone to gain more independence in apartment living, as well as in accessing their community. Staff assist everyone according to their own needs and goals. Skill areas that can be refined are medication management, cooking, shopping, cleaning, organizing, budgeting, increasing natural supports, attendance at religious services and leisure activities, as well as, coordinating outside service provider appointments. This is not a comprehensive list since every plan of care is developed exclusively and uniquely for each individual.

Individuals have a lot of potential for active and productive lives while living at the Jay program. They will plan their own daily schedules with their likes and desires in conjunction with their obligations such as day program, work or maintaining their apartments. In addition, ASI provides different activities throughout the year for all to join such as cooking classes, bingo, and game afternoons. We sponsor agency-wide events such as cooking competitions (Chopped or Chili Cook-Off), Annual Wellness Day Event, sporting events, art classes, and other fun activities to participate in as desired. They will have opportunities to plan activities to do within their home and in their community independently and with staff and peers.
